New Patient Forms
As a new patient with Schmidt Chiropractic Center you will be required to schedule an initial consultation with Dr. Julie to review your past medical history; this time will also allow her to evaluate your condition in detail and work with you on a one on one basis to come up with the best treatment plan for your condition. Please plan up to 45 minutes for your first initial appointment, you will also be adjusted in your first session.
Normally we ask that you arrive 20 minutes early for your appointment to complete necessary paperwork prior to your visit. These forms can be printed on our website ahead of time, they are located below for you to print . We accept several insurances or we offer an affordable time of service discount. If you have your forms completed and ready prior to your appointment simply arrive 10 minutes early.

The selections are based upon if the appointment is for an adult or child, as well as the type of insurance you have or if you are selecting our affordable time of service fee. We also have workers compensation injury and auto accident forms available below too. Simply open the category and print the necessary forms for your appointment.
