Patient Forms

As a new patient with Schmidt Chiropractic Center you will be required to schedule an initial consultation with Dr. Julie,  Dr. Sami or Dr. Brooklin to review your past medical history.  This time will allow the doctor to evaluate your condition in detail and work with you on a one on one basis to come up with the best treatment plan for your condition. Please plan up to 45 minutes for your first initial appointment, you will also be adjusted in your first session.

Normally, we ask that you arrive 20 minutes prior to your appointment to complete necessary paperwork.  If you give us your email address we can send them out and you can complete them before your appointment at your convenience.  We accept several insurances or we offer an affordable "time of service" discount.  We accept cash, check, venmo and most major credit cards. If you have your forms completed prior to your appointment, please arrive 10 minutes early.You can also go to our online booking system at schmidtchiro.janeapp.com to book your first appointmet as well.
